Tips for a tutoring inquiry form that works

Questions

Should students or parents fill this in?

For minors, the parent — they’re the one consenting to lessons and to the contact details being used. For adult learners, the student of course. The form works identically either way.

Can I use one form for several subjects?

Yes — the subject field is free text on purpose. If you only teach two or three subjects, open the template in the generator and change it to a dropdown instead.
