Order form HTML template

A simple order form for taking product orders by email or through your own endpoint — customer details, what they want, how many, and room for special requests. Good for pre-orders, custom work and small shops that don’t need a full checkout.

Tips for a order form that works

• If you sell a fixed set of products, change the Product field to a dropdown so you never get ambiguous orders.
• An order form is not a checkout: never collect card numbers through a plain form. Invoice afterwards, or use a real payment flow.
• Confirm every order by reply email — state price, delivery time and payment terms before treating it as binding.

Questions

Can I take payments with this form?

No — and you shouldn’t try. A plain HTML form must never collect card details. Use it to capture the order, then send an invoice or a payment link. If you need real checkout with cards, you need a store platform.

How do I add my products as a dropdown?

Open this form in the generator with the customize link, change the Product field’s type to Dropdown, and list your products comma-separated.
