Questions

Should I put my prices on the form page?

A starting price ("weddings from $X") next to the form filters out mismatched budgets before they cost you a reply, and makes the inquiries you do get more serious. Full price lists can wait for the follow-up.

How do I stop tire-kicker inquiries?

A required free-text field does most of it — people with a real project write real sentences. The honeypot in this template handles the bots; the textarea handles the humans.
